Photo Contest Entry Form

The City of Irvine marks its 45th anniversary in 2016, and we invite residents and visitors to our community to help us celebrate.  The City's 45th Anniversary Photo Contest aims to capture the essence of Irvine - our neighborhoods and community members, our businesses and innovation, our serene Open Space.  The winners in each of five categories - Outdoors, Community, Historical, Innovation, and 17 and Under - will be revealed in the Fall issue of Inside Irvine.  A grand prize winner will receive a $250 gift card to the Irvine Spectrum Center.  Submissions are due July 1 by 5 p.m. PST. AGREEMENT

IN ORDER TO UPLOAD YOUR PHOTO, YOU MUST AGREE TO ALL OF THE FOLLOWING:

I HEREBY GRANT THE CITY OF IRVINE PERMISSION TO USE MY NAME AND PHOTOGRAPHIC IMAGE SUBMITTED IN CONNECTION WITH THIS PHOTO CONTEST, AS THE CITY IS AUTHORIZED UNDER THE TERMS OF THE RULES OF THE CONTEST. THE CITY OF IRVINE SHALL HAVE ALL RIGHTS, TITLE AND INTEREST TO THE IMAGE(S) AND THE COPYRIGHTS THERETO, AND MAY USE THE LIKENESS AND/OR PHOTOGRAPH, AS APPLICABLE, OR ANY REPRODUCTION THEREOF, IN ANY FORM OR MEDIUM AS IT MAY SELECT.

The City of Irvine takes your privacy seriously. This form asks you to provide the City with certain personal information. Such information is being requested and will be utilized by the City for the specific and limited purpose of future City correspondence regarding the subject-matter of this form. Pursuant to Measure S, an initiative ordinance passed by City voters in 2008, the personal information noted by an asterisk (*) on this form will be kept confidential. Unless you expressly indicate to us otherwise or unless compelled by a court order, it will not be shared with other agencies, businesses or individuals.
